01It is recommended that the address of the Parties corresponds to the address on the certified registration book. If there is another valid address where the Parties wish to be served with the Arbitral documents, please note it below the address space.
02It is recommended that all valid contact information of the Respondent is listed. These include but are not limited to: office telephone number, fax number, e-mail address, cell phone number of the individual in charge.
01The Statement of Claim must be signed by the representative of the Party or by the Power of Attorney
Ordering the Respondent to pay USD 110,000 to the Claimant;
Ordering the Respondent to pay the Claimant interest on any amounts awarded to Claimant at the rate of 15% per annum, from the day following the service of this Request for Arbitration until final payment of any awarded amount;
Ordering the Respondent to reimburse the Claimant for all costs the Claimant incurs in this arbitration including the fees and expenses of the arbitrators, the KCAB International administrative expenses;
Ordering further relief as the Arbitral Tribunal deems appropriate.
01An enumerated list of exhibits should be submitted so that the arbitrator(s) can refer to individual pieces of evidence in the final award.
02Regarding the agency agreement, it is recommended to a) highlight the Arbitration Clause in the contract signed by all of the Parties; and b) check the date of the contract so that the Parties are aware of the Arbitration Rules which apply to the case.
01The Power of Attorney (or an equivalent document) must be signed by the representative of the Party.
02If an employee of the Party attends the hearing instead of the party representative, a proof of employment must be submitted.
03It is recommended that each Party submit a copy of their check or bankbook (including the account number and name of the Party) so that the remainder of costs can be quickly refunded when the case is concluded.
